Practitioners who register and submit a profile agree that:

The information they provide is truthful, accurate, and reflects their actual qualifications and practice
They hold all necessary licenses and credentials in their jurisdiction
They will keep their profile up to date
Their submitted profile is subject to admin review before being published, and may be edited or removed at our discretion
Inclusion in the directory is not an endorsement of their services by Dr. Anthony Chaffee

User-Submitted Content

When you submit content to the site (such as a transformation story, contact message, or practitioner bio), you grant us a non-exclusive, royalty-free, worldwide license to use, display, reproduce, edit, and distribute that content in connection with operating and promoting the site.
